Before your first visit at Wagging Tails Dog Grooming Salon, please fill out the New Customer Form. The more information we have about your dog, the better we can customize the service and treatment to meet everyone’s expectations.
Please share details such as:
Preferences: We each have our own ideas about how we want our dog to appear. However, our expectations and reality may differ, so it's crucial to go over all the details before scheduling an appointment.
Health issues, concerns: If your dog has any health issues, allergies, skin tags, or other skin conditions, it is crucial for us to be informed. If we believe the problem is more serious than initially described, we may recommend consulting a veterinarian.
Coat conditions: Please inform us about your dog's grooming routine, if applicable. Additionally, share how you care for their coat between grooming sessions. We need details about any potential matting and its frequency. If your dog is significantly matted, we may suggest 'shaving'. We will only proceed with your approval, which is why this form is necessary.
Behavioral traits: Understanding your dog's behavior in the grooming environment is crucial for the safety of your dog, our staff, and our equipment. It's important to know if your dog experiences anxiety (such as separation anxiety), is sensitive to noises, struggles with having their nails trimmed, or dislikes having their legs or head touched, or generally finds the grooming process difficult. If this information is inaccurate, it could lead to serious harm to either the dog or the groomer. It is also important to know if you permit us to muzzle your dog (for the safety of your dog, staff, and equipment).
Styling requests (description or photos): If you have a specific style or cut in mind, please inform us, and we will do our best to accommodate your request, prioritizing your dog's health.
Vaccination: Kindly inform us if your dog is fully vaccinated, along with the name and address of their veterinarian.
Photograph(s), Social Media: Please inform us if you agree to have a photograph of your dog taken and shared on our social media pages and website. We may take photos and/or videos of your pet during the grooming appointment. These are stored in a secure, password-protected location. These media are to show current condition, health concerns or a preferred style of coat. If you click 'No' on the form, these photographs will not be shared anywhere.
Parasites: Flea infestation may cause temporary suspension of Wagging Tails Dog Grooming Services, therefore a £10 surcharge will be added to the grooming bill to cover the expense of additional flea-treatment (flea shampoo bath) and disinfection measures required. If ticks are found, a surcharge of £5 will be levied for their removal.
Treats: Kindly inform us if your dog is permitted to have treats. If they are, should the treats be gluten-free or without chicken (or any protein)? Alternatively, would you prefer to bring your own food or decline the offer entirely?
